A. A copy of this plan shall be kept on-site and shall be readily <br />available to all personnel involved with compliance with the <br />plan. <br />B. The Statement of Certification is found on the previous page. <br />C. Records pertaining to implementation of the plan will be kept <br />and made available on request to the Division, CDMG, and EPA <br />representatives. Records shall be kept in accordance with all <br />applicable requirements as explained in Part I C.4., page 14, <br />CDPS General Permit. <br />D. Personnel directly responsible for stormwater management are <br />Juan Garcia or Jim Kiger or Rich Atkinson or other duly <br />authorized representative. These designee's will be familiar <br />with this plan in its entirety, and therefore are capable of <br />judging compliance with the plan. <br />E. Proper training shall be provided for affected employees in <br />techniques and procedures that will assure achievement of all <br />stated goals. <br />F. This plan shall be amended at any time if there is a change in <br />operation that will or may result in a deterioration in the <br />quality of pollutants addressed by this plan, or if the plan <br />is shown to be ineffective in achieving the specified goals. <br />A copy of the amended plan shall be submitted to the Division <br />and the DMG within 30 days, along with an explanation of the <br />reason for the amendment. If the Division finds the amended <br />plan to be inadequate, or if the procedures established are <br />shown to be ineffective once in place, new procedures shall be <br />developed and employed. <br />G. An annual report will be submitted, due on the anniversary <br />date of the facility certification, July 1. The report will <br />be signed and certified by a principal executive officer of at <br />least the level of vice-president, or his duly authorized <br />representative, and will include a statement affirming that <br />thorough quarterly inspections of all affected areas covered <br />under this plan, have occurred. If the inspections detect <br />complete conformance with the plan, this shall be so stated. <br />If the inspections show the plan to be inappropriate or <br />ineffective in any area, the annual report shall address how <br />H-G has corrected or intends to correct this deficiency. All <br />annual reports shall become a part of this permit by <br />reference. <br />H. Whenever a facility or a portion of a facility enters the post <br />mining phase, it is possible that the overall configuration of <br />the site may change. Adequate measures shall be taken to <br />minimize any erosion that may occur. All facilities at the <br />H-G mine site have been previously reclaimed. <br />
