Weinman , da[ed July 13, 1999, 1 am sending a termination form which must be <br />completed and signed by you, as you are listed as the legal contact for the above-referenced permit. Before the Division can <br />consider inactivating the above-referenced permit, a release of bond from Division of Minerals and Geology is required, <br />At that time you will need to submit the enclosed form along with a copy of the bond release from the Division of Minerals and <br />Geology. Also, you must certify tha[ all discharges to state waters have ceased. Upon receipt of the completed termination <br />form, the District Engineer (D.E.) for the above-referenced county will be notified and will inspec[ your facility. Regulations <br />require that monitoring reports be submitted until the permit is formally inactivated. If appropriate, such reports should <br />simply state "no discharge". <br />Please be aware that Section 6.16.0(5) (B) of the permit regulations state: <br />"Once the Division proceeds to [erminate a permit a[ the permittee's request, the prorated fee shall apply to the <br />period of time the permit has been in effect, including but not exceeding ninety (90) days from the date the permit <br />termination request is received by the Division." <br />Please contac[ me at (303) 692-3599 with any questions contenting fees.
