27. Within one hundred and fifty (150) calendar days ofreceipt of this Order, Gold King shall develop and <br />submit to the Division a Facility Operations and Maintenance Manual. At a m;n;mum the Operations <br />and Maintenance Manual shall include the following: <br />a. A written description of the personnel requirements and qualifications for the operation and <br />management of the Facility, including a description of each individual's role, duties and responsibility <br />in ensuring the proper operation and maintenance of the Facility, <br />b. A copy of the Facility's records retention policy and/or procedures, which should specifically <br />reference the following types of records; maintenance records, equipment inventory records, daily <br />operation logs and both process and regulatory monitoring (sampling/analysis) records. <br />c. A detailed description of the treatment processes utilized by the Facility, including, but not limited to, <br />a description of each treatment process (Precipitation, Coagulation, Flocculation, pH control; ...etc.), <br />operations, controls and process flow schematics and as built design drawings. <br />d: A detailed description of the utility systems supporting the Facility, including backup systems. <br />e. For each element of the manual that describes equipment, the manual should include copies of <br />equipment operation manuals, user guides, preventive maintenance /trouble shooting procedures, a <br />list of onsite replacement parts and the part storage location, and tune schedules for the completion of <br />each required maintenance activity. <br />f. A detailed description of the Facility's compliance monitoring sampling/analysis procedures and <br />monitoring schedules. <br />g. A detailed description of the Facility's process testing and samplinglanalysis procedures. <br />h. Facility upset or emergency operation procedures, which include the following: <br />i. An outline of the roles and responsibilifies ofthe Facility operators during emergency <br />operations. <br />u. .Criteria for implementing emergency operations. <br />iii. Internal and external notification procedures, including notification to the Division and <br />downstream water users. <br />i. Operator health and safety information and procedures. <br />28. Beginning sixty (60) calendaz days from receipt of this order, and every sixty (60) calendar days . <br />thereafter, until such time as this Order is closed or the Division decrees so in writing, Gold King shall <br />submit written progress reports to the Division.
