Before the Division <br />can consider inactivating the above-referenced permit, a release of bond from Division of Minerals and Geology is <br />required. A[ [hat time you will need to submit the enclosed form along with a copy of the bond release from the Division <br />of Minerals and Geology. Also, you must certify that all dischazges to state waters have ceased. Upon receipt of the <br />completed termination form, the District Engineer (D.E.) for the above-referenced county will be notified and will inspect <br />your facility. Regulations requve that monitoring reports be submitted until the permit is formally inactivated. If <br />appropriate, such reports should simply state "no dischazge". <br />Please be aware that Section 6.16.0(5) (B) of the permit regulations state: <br />"Once the Division proceeds to terminate a permit at the petmittee's request, the prorated fee shall apply [o <br />the period of time the permit has been in effect, including but not exceeding ninety (90) days from the date the <br />permit termination request is received'oy the Division.' <br />Please contact me at (303) 692-3599 with any questions concerning fees.
