Outfalls 003A and OOSA dischargetl each week of the period. Outfall 006A did not discharge the last week of <br />the period. <br />In order to reduce the amount of paper work, the following certification will be substituted for the actual DMR forms to cover <br />the outfalls, which did nol discharge. These outfalls include pontls T (Outfall 002A), C (Outfall 004A), F (Outfall 007A), and G <br />(Outtall OOSA). Note that ponds A (Outfall OOtA), and H (Outfall 009A) have been deactivated following bond release, and <br />have been removed from the newly amended permit, effective January 1, 2003. Also note that Outtall 020A has not been <br />constructed yet. <br />CERTIFICATION <br />I certify under penalty o/law that 1 have personally examined and am familiar with the in/ormation submitted herein; and <br />based on my inquiry of those individuals responsible for obtaining the information, I believe the submitted information is <br />true, accurate, and complete. 1 am aware that there are significant penalties for submitting false information, including the <br />possibility of fine and imprisonment. See 18 U.S.C. § 1001 and 33 U.S.C. § 1319.
