<br />C>!UALITY ASSURANCE PLAN <br />J <br />5.3 Sample shipping <br />Some clianl contractual agreements require that our lab ship a duplicate sample <br />to another laboratory, or to forward reference sample material to the fieltl, office, <br />or again another laboratory. <br />Samples are shipped via chain-of-custody in clean Igloo~ype coolers packed in <br />blue ice using over light express or next~ay delivery mail. <br />A follow-up telephone call is placed the following work day to confirm sample <br />receipt and sample condition. <br />6.0 CHAIN-OF-CUSTODY DOCUAIENTATIDN <br />If requested by client, Fr4S will maintain an unbroken record of possession of a <br />r-- sample from the time of its collection through some analytical or testing <br />procedure, and when necessary, up to, and through, a court proceeding, by the <br />use of achain-of-custody form- <br />This laboratory does not normally follow strict chainirf-custody procedures In <br />handling routine samples received for iesting or analysis. 1-t1e submittar is <br />usually responsible far determining whether chain-of-custody procedures are <br />necessary for a particular sample or sample set. Usually chain-0f-custody <br />documentation is necessary when laboratory results are to be used as evidence <br />in legal proceedings This documentation is prepared in addition to the normal <br />sample processing paperwork. <br />Otherwise, samples are received, bgged-in, and a record of rocpipt to client, <br />handling, storage and tlisposal of sample is provided to client. <br />Client samples era stored in a locked refrigerator end are Haver removed from <br />ih9 EAS Division of Sangre De Cristo Laboratory, Inc. without a signed chain-of- <br />custody reflecting "transferred to" and "accepted by" ertities. <br />60 3Jtld ~QS L69E6856ILT 88=90 466LlELli±O <br />
