Please type the title of each exhibit at the <br />number the pages of Exhibits A-F consecutively for <br />sections describe the minimum information that must be <br />EXHIBIT A - Legal Description <br />The legal description must identify the affected land, specify the acreage of affected <br />land and be adequate for field location of the property. It should be a description by <br />reference to township, range, and section, to at least the nearest quarter-quarter <br />section, A metes and bounds survey description is also acceptable. Where applicable, <br />the street address or lot number(s) may be used. This information is available from the <br />County Assessor's office or maps from the U.S. Geological Survey (USGS). ' <br />EXHIBIT B - Site Description <br />Items 1-3 below, must be addressed to the extent necessary to .demonstrate compliance <br />with the applicable performance standard requirements of Rule 6. At a minimum, you <br />should include the following information. <br />1. A description of the vegetation and soil characteristics in the area of the proposed <br />operation. The local office of the Soil Conservation Service (SCS) will provide you <br />with this information as well as recommendations for Exhibit D - Reclamation Plan. <br />Contact the Regional SCS office in Denver at (303) 964-0295 or the Mined Land <br />Reclamation Division for assistance in locating the office nearest you. <br />2. A statement prepared by a representative from the local office of the Colorado; <br />Division of Wildlife or other qualified individual describing the species and habitat of <br />wildlife that might be affected by the proposed operation and measures to be utilized to <br />provide for the safety and protection of wildlife, critical habitat or species. Cgntact <br />the Division of Wildlife office in Denver at (303) 297-1192 or the Mined Land <br />Reclamation Division for assistance in locating the office nearest you. <br />3. Identify ar4Y permanent man-made structures within 200 feet of the affected area and <br />the owner of each structure. Permanent man-made structures include houses, barns,: <br />fences, above or below ground utilities, irrigation ditches, maintained or public roads, <br />bridges, wells, pipelines, water storage impoundments, railroad tracks, cemetaries; and <br />carcnunication antennas. Each structure should be located on Exhibit E -Map. <br />
