Lang <br />Re: File No. M-2000-098, Monarch Quarry, 112c Decision Leller -Conditions, Division Director <br />Financial and Performance Warranty Request -Construction Material Operation <br />Dear Mr. Bowers: <br />On January 9, 2002, the Division of Minerals and Geology approved your 112c mining permit application. The conditions 10 <br />the approval are noted below: <br />Stipulation No. Description <br />Once specific areas have been identified within the permit boundary where limestone and dolomite are to be quarried (mined), <br />the Operator shall submit a revision [o Ute permit, to the Division, prior [o conducting any quarry operation for limestone or <br />dolorttite. The revision shall be a Technical Revision if the curently proposed mining and reclamation plans are not being <br />altered (changed). The Technical Revision shall include an accurate mining plan map showing the direction of mining and an <br />accurate reclamation plan map showing the expected topography of the reclaimed lands. The maps must accurately delineate <br />the limestone and dolomite areas. In addition, the exhibit maps must comply with requirements of Rule 6.1. If the operator <br />proposes to significantly change the mining plan, which may significantly change the reclamation plan, in the limestone and /or <br />dolomite areas, an Amendment to the permit must be submitted to the Division. <br />The amount of financial waranty set by the Division for this operation is $209,000.00. A copy of the cost summary form is <br />enclosed for your review. You must submit a financial waranty in this amount and a performance waranty in order for us to <br />issue a permit. In the event you have requested a financial wartanry forth, we have enclosed it in this letter. If you have not, <br />please select a type of financial warranty from Rule 4.3. Then contact us so that we can provide you with the appropriate <br />warranty form. We have enclosed a performance warranty forte with this letter for your use. <br />PLEASE NOTE THAT MINING OPERATIONS MAY NOT COMMENCE UNTII, A PERMIT HAS BEEN ISSUED <br />BY THE DP/LSION AFTER RECEIPT OF YOUR FINANCIAL WARRANTY AND PERFORMANCE <br />WARRANTY.
