Banks and Gesso, LLC <br />EXHIBIT M -OTHER PERMITS AND LICENSES <br />Additional permits/approvals required for the Haldorson property are listed below. <br />Montrose County Use by Special Review Permit <br />An application for a Use By Special Review Permit is being submitted concurrently with <br />this CDMG 112 Permit application to Montrose County, Colorado as part of the land use <br />permission granted to mine the Haldorson property. <br />CDPS State Discharoe Permit/Stormwater Permit <br />Prior to any ground-disturbing activities at the proposed permit area, a <br />CDPS/Stormwater Permit will be obtained from the Colorado Department of Public <br />Health and Environment (CDPHE) that will account for any and all off-site discharge and <br />will contain a plan for control of that discharge. <br />Air Permits <br />Prior to any ground-surtace disturbing activities (topsoil removal and stockpiling, access <br />road development, haul road construction or material hauling) for the Haldorson <br />property, the Applicant will obtain an Air Pollution Emissions Notice (APEN) from the <br />CDPHE. This permit will be kept active during the entire life of the mine. <br />Road Access and Maintenance Agreement <br />Access to the site will be from a location on Road 6175 located approximately 600 ft <br />south of LaSalle Road. Haul trucks will exit the site from this point and travel north to <br />LaSalle Road where they will then travel eastbound to U.S. Highway 50. From U.S. 50 <br />the trucks will then travel south to the City of Montrose.
