Enclosed is a form which need to be completed by you as legal contact prior to the <br />Division terminating the coverage of this site under the general permit. <br />Please complete the enclosed inactivation form. If an area is not applicable, please so <br />indicate. Based on the information you have provided to the Division thusfar, we anticipate <br />that we can inactivate your entire permit. <br />Upon receipt of the completed form, the District Engineer for the above-referenced county <br />will be notified and will inspect your facility. Regulations require that monitoring reports <br />be submitted until the permit is formally terminated. If appropriate, such reports should <br />simply state "no discharge". <br />Please be aware that Section 6.16.0(5) (B) of the permit regulations state: <br />"Once the Division proceeds to terminate a permit at the permittee's request, the <br />prorated fee shall apply to the period of time the permit has been in effect, including <br />but not exceeding ninety (90) days from the date the permit termination request is <br />received by the Division." <br />
