A letter of certification containing the results of the inspection must be submitted to the Division within two weeks following the inspection." In accordance with this stipulation, I, Thomas J. Wainwright, Colorado P.E. 28482, inspected the subject embankment on June l0, 1993 and do herewith submit my report of findings: I. Chronology A. July 14, 1981: F.M. Fox and Associates submitted a report on their geotechnical slope stability investigation of the embankment. The report concluded that the embankment was stable with a minimum static safety factor of 1.52. The report also recommended revegetation and inspections. B. September 23, 1981: Colorado Mining Permit No. C-80- 005 became effective. C. Fall 1981: The embankment was hand seeded in compliance with Stipulation 16 of Permit No. C-80-005. D. June 15, 1982: The annual embankment inspection was conducted. Weather conditions were favorable, skies partly cloudy, temperature about 65° F. Alight rain had fallen on the evening of June 14th.
