M-96-008, RECIIPT OF REGULAR 1 l2 CONSTRUCTION <br />MATERIALS RECLAMATION PERMIT APPLICATION PACKAGE -INADEQUATE <br />Dear Ms. Hovey: <br />On March 28, 1996, the Division of Minerals and Geology received your application package <br />fora 112 Construction Materials Reclamation Permit for the North R-34 Pit, File No. <br />M-96-008. Preliminary review of the information received determined [hat the following <br />information must be provided before the Division can consider your application submitted and <br />can begin the technical review: <br />1) Item fi 11, post-mining land use, and 1l 12, present land use. <br />2) Exhibit O -Proof of Mailine Notices to County Commissioners and Soil Conservation <br />District: It cannot be determined what sort of notices were submitted to the Montrose <br />County Commissioners. The appropriate notice forms are provided with the application <br />form. <br />3) Exhibit S -Permanent Man-Made Structures: A copy of Rule 6.4.19, which became <br />effective on January 2, 1996, is enclosed. You submitted an older version of the <br />required application form which did not list all the currently required exhibits. <br />You may amend your application by submitting the information fisted above. In accordance with <br />Rule 1.8, a new decision date will be established 90 days from the date of receipt of all of the <br />requested information. Additionally, if you have already published notice, for this application, <br />you will need to republish notice, after the Division considers your application submitted. This <br />public notice shall be published four times, once a week for four consecutive weeks, beginning <br />within 10 days of the date the application is considered submitted. The fmal date for receiving <br />comments is ten (30) days after the date of publication or the next regular business day. <br />
