No permit limitation exceedences occurred during this quarter. <br />I have not yet received current DMR forms, as requested from Leslie Simpson. However, she had indicated that we could use <br />a replicated torm for submittal. <br />Chronic Whole Effluent Toxicity (WET) testing were performed at Outialls 003, 022 and 024, indicated no toxicity to P. <br />promelas or C. dubia. The "Concentration with Statistically Significant Difference in Lethality" was >100% for both species at <br />each Ouffall. Copies of the CDPS WET Test Repon Forms for both species are included in the submittal. <br />Outfalls 004, 007, 009, 011, 012, 013, 014, 015, 016, 017, 018, 019, 020, 021, 023 and 024 along with Site 3001 did not <br />discharge during this period. In order to reduce the amount of paper work, the following certification will be substituted for the <br />actual DMR forms to cover the outfalls which did not discharge. <br />CERTIFICATION <br />I certify under penalty of law that I have personally examined and am familiar with the information submitted heroin; and <br />based on my inquiry of those individuals responsible for obtaining the information, 1 believe the submitted in/ormation is <br />true, accurate, and complete. I am aware that there are signi(canf penalties /or submitting /alse information, including the <br />possibility o/fine and imprisonment. See 18 U.S.C. § 1001 and 33 U. S. C. § 1319.
