A <br />copy of `as-built" well diagram and EPA form (Plugging and Abandonment). 7520- <br />14 is also enclosed. The plugging and abandonment (P/A) plan meets or exceeds <br />the requirements in the Mine Plan and the UIC Permit. The proposed activity will <br />isolate aquifers and verify cement. Well 4A-2V was the northern most recovery <br />well for cavity #4. Because of drilling problems near first recovery well- 4A-1 V, <br />the horizontal connection and therefore mining was never performed. We114A-2V <br />was cased and had cement returns to surface. A CBL shortly after completion (2- <br />10-95) indicated good cement top at 1532 FFS, with poor cement fill above 1532 <br />FFS. P/A work for 4A-ZV is planned to coincide with 91-1 V and 93-3V P1A. <br />In summary, a cast iron bridge plug (CIBP) will be set; a cement plug will be <br />installed from CIBP to top of annular cement. Cement will be tagged, casing <br />perforated and cement squeezed. Cement will be verified with cement bond log <br />and procedure repeated until aquifers are isolated. Finally, the annulus and well <br />bore will be cemented from surface and a surface hole location marker will be <br />installed at grade. The P/A work is planned in May, with contouring and <br />reclamation in October 2005.
