Box 299 <br />Silverton, CO 81433 <br />Reference: Inspection Mogul Mine bulkhead preparations <br />Dear Steve: <br />This letter report presents my inspection of the No. 1 Level <br />drainage tunnel bulkhead form for the Mogul ]Kine on August 19, <br />2003. The bulkhead form and other preparations meet all the <br />design criteria. Filling of the bulkhead foam can proceed when <br />the concrete is available. <br />I arrived at the bulkhead location at approximately 2:00 p.m. <br />and entered the bulkhead for the inspection. All of the #8 bars <br />had been tied into the downstream (airside) cage and the #6 bars <br />had been tied into the upstream (waterside) cage at the minimum <br />3.5 inches from the plywood form faces. Only the final three <br />horizontal bars had not been tied into the downstream cage in <br />order for final access for cleaning and initial filling of the <br />form. The horizontal bars were hanging abovf=_ the access and <br />inspection opening, ready for positioning when the concrete pour <br />reaches that elevation. Two vertical bars that had been spread <br />sideways for access to the form and were tied into the cage near <br />the floor. They will be pulled back and tied into the cage as the <br />concrete rises in the form. <br />All the loose rock on the perimeter of t:he 9-ft thick <br />bulkhead location between Stations 2+91 and <?+50 had been <br />completely scaled, the resulting rock surface' was clean exposing <br />fresh rock everywhere, the 6-in diameter stainless steel bypass <br />pipe complete with the combination thrust plate waterstop <br />installed, two additional waterstops were installed along the <br />bypass pipe, the waterside and airside forms had been erected and <br />the reinforcing rebar cages tied in place. <br />The dimensions of the bulkhead forms were checked during the <br />inspection. The thickness of the bulkhead i:: 9-ft, as designed. <br />Scaling of the loose rock had increased the cimensions of the <br />1 <br />
