<br />~ ~ • <br />Mr. William Thurston - 3 - Octo~er 31, 1985 <br />EXHIBIT E - Map <br />You must provide a map that clearly describes the features associa$ed with the <br />mining plan and describes the components of the reclamation plan. You may <br />want to include one map for the mine plan and one map for the reel mation <br />plan. The map(s) should be drawn to a scale no smaller than 1 inc~ = 50 feet, <br />show a north arrow and show locatable points adjacent to t e propo e <br />operation. At a minimum, you should include the following informa7lion. <br />Mining Plan <br />1. Outline and label the permit boundaries and the affected area escribed in <br />Exhibit A - Legal Description. <br />2. Label the names of owner(s) of record of the surface of the af~ected area <br />and of the land immediately adjacent to the affected area. <br />3. Outline and label all major surface features to be used in con ction with <br />the proposed operation such as: existing and proposed roads, pit oundary, <br />topsoil stockpiles, overburden stockpiles, product stockpiles, was a rock <br />fills, stream channels, buildings, processing plant, ponds, impoun ents, <br />diversions, waste disposal areas. <br />4. Indicate the direction that mineral extraction will proceed. <br />5. Note the location of any significant, valuable, and permanent n-made <br />structures within 200 feet of the affected area such as: houses, b rns, <br />fences, above or below round utilities, irrigation ditches, mainta'ned or <br />public roads, bri ge~1 swipe Ines, water storage impoundments, railroad <br />tracks, cemeteries, and communication antennas. A narrative descri tion <br />should be provided in Exhibit B - Site Description. <br />6. Outline and label existing disturbance, if any, within and adja ent to the <br />permit boundary (e.g., previously mined areas, roads or excavations resulting <br />from utility construction). If you redisturb areas that have been re viously <br />disturbed, then they must be included in the permit area and addre sed in <br />Exhibit D - Reclamation Plan. <br />Reclamation Plan <br />1. Indicate the gradient of all reclaimed slopes (horizontal:verti al ). <br />2. Label the location where vegetation will be planted. <br />3. If ponds are a part of the reclamation plan, outline the final ho re <br />configuration of the ponds. <br />4. Note the thickness of replaced overburden. <br />5.
