Paee 2 of 19 <br />Permit No. COR-040000 <br />CDPS GE\~RAL PERMIT <br />STORMWATER DISCHARGES ASSOCIATED WITH <br />bLETAL NIINL~G OPERATIONS A\'D i1~~WASTE RE~fEDL~.TIO\ <br />AUTHORIZATION TO DISCHARGE UNDER THE <br />COLORADO DISCHARGE PERMIT SYSTEM <br />In compliance with the provisions of the Colorado Water Quality Control Act, (25-8-101 et seq., CRS, <br />1973 as amended) and the Federal Water Pollution Control Act, as amended.(33 U.S.C. 1251 et seq.; <br />the "Act"), this permit authorizes the discharge of stotmwater associated with metal mining operations <br />and mine-waste remediation certified under this permit, from those locations specified throughout the <br />State of Colorado to specified waters of the State. Such discharges shall be in accordance with the <br />conditions of this permit. <br />This permit specifically authorizes the facility listed on page 1 of this permit to discharge stormwater <br />associated with mining operations or mine-waste remediation, as of this date, in accordance with permit <br />requirements and conditions set forth in Parts I and II hereof. All discharges authorized herein shall be <br />consistent with the terms and conditions of this permit. <br />This permit and the authorization to discharge shall expire at midnio-ht, September 30, 2001. <br />Issued and Signed this day of AUG 2 0 1996 <br />COLORADO.DEPARTMENT OF HEALTH <br />J.
