MONITORING REQUIREMENTS <br />2. ge~ortine of Data <br />Reporting of the data gathered in compliance with Part I.B.1 shall be on a Quarterly <br />basis. <br />Monitoring results shall be summarized for each calendar quarter and reported on <br />Division approved discharge monitoring report (DMR) forms postmarked no later than <br />the 28th day of the month following the end of the quarter (fox example, the DMR for <br />the first calendar quarter must be postmarked by April 28th). If no discharge <br />occurs during tha reporting period, "No Discharge" shall be reported. <br />~I <br />The first o a signed copy of each discharge monitoring report (DMR) shall be <br />submitted to the Division at the following address: <br />Colorado Department of Health <br />Water Quality Control Division <br />Permits and Enforcement Section <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80222-1530 <br />The first duplicate signed copy of each discharge monitoring report (DMR) shall be <br />• submitted to the following agency: <br />U.S. Environmental Protection Agency <br />Water Management Division <br />NPDES Branch 8WM-C <br />999 18th Street, Suite 500 <br />Denver, CO 80202-2466 <br />Compliance Branch 8WM-C <br />3. Storm Exemption - Discharge points 007, 009, 010 and 012 <br />If Che permittee intends to use the 10-year, 24-hour storm exemption, a report must <br />be submitted within 90 days after the effective date of this permit, documenting <br />that the facility is designed, constructed and will be maintained to contain or <br />treat: <br />(a) The maximum volume of wastewater which would be generated by the facility <br />during a 24-hour period without an increase in volume from precipitation; and <br />(b) The maximum volume of wastewater resulting from a 10-year, 24-hour <br />precipitation event, including the volume which would result from all areas <br />contributing runoff to the individual treatment facility (i.e., all runoff not <br />diverted from the active mining area or the mill area). <br />• <br />Amendment 1 Issued 3/LO/93 Effective 5/1/93 <br />
