COVERAGE UNDER THIS PERMIT <br />~1 <br />• This general permit authorizes dischargeserarions including loadouts while lactive mining isooccurring or during re larnation prior to <br />ferruginous coal mining and processing op <br />final bond release, to land (as dust control), and into waters of the State of Colorado. If additional sources which the Division determines <br />to be minor in nature such as selected mine water, prepazation plant water, spoils spring water, runoff through refuse piles, or other <br />wastewater may be dischazged. These sources tnay be covered and Le~~eme is other thanrthose contained ~m this permitat Such <br />wastewater would not require effluent limitations and/or monitoring q <br />determinations will bemade on acase-by case basis upon review of the pernilt application and any additional relevant information. This <br />pemilt does not cover underground mining. If dischazge from one or more of the above listed sources require additional limitadons <br />and/or monitoring requirements, an individual permit maybe required. Multiple dischazge points from one facility maybe authorized I <br />through this permit providing all meet these criteria. <br />2. In order to apply for ceRification under this general permit, the ownerwoastewater D~scharge App 1 ationt (fumishedlby the Divisionl) <br />submit by certified mail or hand delivery, the completed Coal Mining <br />which generally requires the following information (at a minimum): <br />a. Name, address, and descriptive location of the facility; <br />b. Name of principal in charge of operation of the facility; _ lrcable <br />c. Name of itrtmediatereeeof agtivitersesincsnlbng mgthe disctharge, including the anticipated trtationgof the activity and/or the <br />d. Description of the typ tYC ) <br />discharge, anticipated volume and rate of discharge, and the source(s) of water to be discharged; <br />e. Description of any wastewater treatment system(s) and recycle/reuse utilized; <br />f. An 8 Yn x 1 l inch map showing the general geographical location of the facility and any neazby landfills, mine or mill tailings, or any <br />other known or suspected sources of pollutants; <br />g. An 8 %: x 11 inch sketch of the facility showing all structures, outfalls and receiving waters, as well as storage locations o any <br />petroleum or chemicals on site; and <br />h. A chemical analysis of the water to be discharged. If no water is available, this shall be indicated. <br />If the discharge is to a storm sewer system, ditch, or other manmade conveyance, approval from the owner of the systemmust be obtained <br />before dischazge. <br />At ]east 30 days bef°re the anticipated date of dischazge, such information shall be submitted on the application form to: <br />Colorado Department of Public Health and Environment <br />Water Quality Control Division <br />WQCD-P-B2 <br />4300 Cherry Creek Drive South <br />Denver, CO 80222-1530 <br />The Division shall have up to thirty days after receipt of the above information to request additional data or deny the authorization to <br />discharge. Upon receipt of additional information, the Division shall have an additional 30 days to issue or deny authorization to <br />dischazge.
