Reoortine of Data <br />Reponing of the data gathered in compliance with Part I.B.1 shall be on a vuarterly basis. Monitoring results shall be <br />summarized for each calendar quarter and reported on Division approved discharge monitoring report (DMR) forms <br />(EPA form 3320-L). The farms shall be mailed to the agencies listed below so they are received no later than the 28th <br />day of the month following [he end of the quaver (for example, the DMR for the frsr. calendar quaver must be received <br />by the Division by April 28th). if no discharge occurs during the reporting period, "IVo Discharge" shall be reported. <br />The DMR forms consist of four pages -the tap "original' copy, and three attached no~azbon-required copies. After the <br />DMR form has been filled out and signed, the four copies must be sepazated and distributed as follows: <br />The First orieinal signed copy of each discharge monitoring report (DMR) shall be submitted to Ore Division at the <br />following address: <br />Colorado Department of Public Health and Environment <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Denver, CO 80222-1530 <br />The first duplicate signed copy of each discharge monitoring report (DMR) shall be submitted to the following agency: <br />U.S. Environmental Protection Agency <br />Technical Enforcement Program 8ENF-T <br />Office of Enforcement, Compliance Assistance and Environmental Justice <br />999 18th Street, Suite 500 <br />Denver, CO 80202-2466 <br />The third and fourth copies are for the permittce records. The Discharge Monitoring Report forms shall be filled out <br />accurately and completely in accordance with requirements of this permit and the instructions on the forms. They shall <br />be signed by an authorized person as identified in Part I.D. <br />3. Chronic WET Testine-Outfalls 003. 022 avd 024 <br />(a) Tes[ine and Reoortine Requirements <br />Test results shall be reported on the Discharge Monitoring Report (DMR) submitt~M for the reporting period during <br />which the sample was taken. (i.e., WET testing results for the calendaz quarter ending March 31 shall be reported <br />with the DMR due April 28.) The results shall also be submitted on the Chronic Tonicity Test report form, <br />available from the Division. Copies of these reports are to be submitted to both dte Division and EPA along with <br />the DMR. <br />The permittee shall conduct each chronic WET test in general accordance with methods described Short Term <br />Methods for Esthnatine the Chronic Tonicity of Effluents and Receivine Waters to Freshwater Oreanistns, <br />EPA/Ci00/4-89/OOL or most current editions, except av modified by the most current Division guidance document <br />entitled Guidelines for Conductive Whole Effluent Toxicity Tests.
