Evidence <br />is attached to this certification that: <br />Municipalities within athree-mite radius of the proposed alteration or relocation have been <br />notified in writing of the proposed alteration or relocation. <br />2. The Colorado Water Conservation Board has been notified in writing of the proposed alteration <br />or relocation. <br />3.
