DEPAR7.'MENT EVALUATION:
Based on the information provided and the Department's review,
the application is acceptable for review.
ITEMS FOR CONSIDERATION:
1. Noti ication, by certified mail, return receipt requested,

Planning & Zoninq with copies of any technical revisions to
its Mined Land Reclamation permit, and with any copies of
any and all annual or other mining reports required by any
governmental agency.

Conditional Use Permit will result in termination of the

with copies of all renewed leases prior to expiration.
