Under Part II Section C.1. ^Prior To Commencing <br />Injection" of UIC Area Permit No. C03858-00000 (the Permit), <br />injection operations may not commence into any newly drilled or <br />converted well(s) until the Permittee has received written <br />authorization from the EPA as to the following: <br />1. Construction is complete and the Permittee has <br />submitted information to the Director that includes EPA <br />Form 7520-10, a schematic well bore diagram after <br />construction, and detailed drilling, construction and <br />cementing records including any open hole tests; and <br />2. copies of all pertinent logs run during the drilling <br />and completion of the well including a Cement Bond Log <br />(CBL), accompanied by a narrative interpretation by a <br />knowledgeable log analyst, have been submitted; and <br />3. the mechanical integrity of the well has been <br />demonstrated and the results of a successful pressure <br />mechanical integrity test submitted; and <br />4. formation data and injection fluid analyses have been <br />submitted. <br />~Pdnted on Recycled Paper <br />
