In order to reduce the amount of paper work, the following certification will be <br />substituted for the actual DMR forms to cover the outfalls, which did not discharge. These outtalk include ponds T (Outfall <br />002A), B (Outfall 003A), C (Outfall 004A), D (Outfall OO5A), E (Outfall 006A), F (Outiall 007A), and G (Outiall OOSA). Note that <br />ponds A (Outfall 001 A), and H (Outfall 009A) have been deactivated following bond release, and have been removed from the <br />newly amended permit, effective January 1, 2003. Also note that Outfall 020A has not been constructed yet. <br />1 certi/y under penalty o/law that 1 have personally examined and am familiar with the information submitted herein; and <br />based on my inquiry of those individuals responsible for obtaining the information, 1 believe the submitted information is <br />true, accurate, and complete. 1 am aware that there are significant penalties forsubmitting /a/se information, including the <br />possibility of fine and imprisonment.
