General ~~ <br />1. Rule 6.2.1 (2)(b) requires that maps except the index map be prepared and signed by a registered land <br />surveyor, professional engineer or other qualified person. Your Exhibit E- maps have an approving <br />person's initials but no signature of the preparer. Please provide the signature. <br />2. Rule 6.2.1(2)(e) requires [hat the Exhibit E maps have a title. Please change the titles to Mining plan <br />Map and Reclamation Plan Map as required by Exhibit E. <br />Exhibit A-Legal Description and Location Map <br />1. Rule 6.3.1(1) requires that the legal description identify the affected land and specify affected areas. <br />The description must be by township, range and section to the nearest quarter quarter~uauersection. <br />Your description describes a parcel of land of 378.46 acres whereas your application is for a permit area <br />of 9.9 acres. Please describe the permit area to at least the nearest quarter quarter section (40 acre <br />parcel) <br />Exhibit B-site Description <br />In accordance with Rule 6.3.2 a provide a description,such as a Natural Resource Conservation Service <br />range site description, of the native rangeland vegetation. Provide at a minimum a list of the major <br />important species and an estimate of their percent of the total vegetative composition. The local NRCS <br />office may provide you with this information. <br />2. You have stated that there are no stmctures within the affected area of the site. Rule 6.3.2(6) requires <br />that you identify any permanent man-made structures within 200 feet of the affected azea and the owne <br />o~ch strt~L[uye. ~d~ntif~ny signifi~an~valuable permanent structures,t wi~~ci~:- y~o ~,°~ o <br />.a.~ cTe~ sh bOUti.iC <br />3. Also in accordance with Rule 6.3.2(a) provide a description of the s[oneham Sandy Loam soil. The <br />description can be obtained from the NRCS. <br />Exhibit C-Mining Plan <br />1. State the estimated depth to which soil, suitable, as a plant growth medium will be salvaged for use in <br />the reclamation process. (see Rule 6.3.3(6)). The soil salvage depth must be consistent with <br />information provided in Exhibit B (see Exhibit B 3. Above) <br />2. Specify the thickness of overburden, if any, to be removed to reach the deposit. The location of any <br />overburden stockpiles must be shown of the Exhibit E Map. See exhibit 6.3.3(d). <br />3. Specify the thickness of the gravel resources to be mined. See exhibit 6.3.3(d). <br />4. Does the haul road exist now? If it does and you do not intent to up-grade it, add the information to this <br />exhibit. If it does not presently exis[ryou must add the road to the area you are requesting to be <br />permitted . Since your present permit area is for 9.9 acres, the haul road can only be added if it will <br />disturb less than 0.1 acre. This would allow the permit application to remain under the 10 acres required <br />by law. (See exhibit 6.3.3(8)) <br />5. Will any water be used in conjunction with the operation? L` so, what is the source of the water? (Rule <br />6.3.3(h)) <br />6. The proposed operation is close to a spring and an intermittent stream. Will the excavation intercept <br />(expose) groundwater? Will the excavation daylight to the East? (allow drainage from the pit to reach <br />
