Note that only Pond D {Outtall OOSA) discharged to Foidel Creek in June, <br />and that was only briefly during the middle of June, allowing only one sample set to be collected for the month, <br />Ouffalls 002A (Pond T), 003A (Pond B), 004A (Pond C), 006A (Pond E), and 007A (Pond F), and 008A (Pond G) did not <br />discharge in June, therefore no water quality tests were conducted at these sites, although quarterly conductivity results <br />and total dissolved solids (7DS) calculations are reported herein for Outfalls 002A, 004A, OO6A, and 008A. Outfall <br />020A/020W has not yet been constructed. Therefore no sampling of this site has occurred. Note that ponds A (Outtall <br />001A/OO1W), and H (Outtall 009A) have been deactivated following bond release, and have been removed from the amendetl <br />permit, effective January 1, 2003. In order to reduce the amount of paper work, the following certification will be substituted for <br />the actual DMR forms to cover the ouffalls, which did not discharge. <br />Quarterly W hole Effluent Toxicity (WET) testing was conducted for Outtall OOSW in mid June 2006. This test indicated that the <br />effluent were not acutely toxic to Daphnia magna survival (see attached laboratory report). Outtall 007W did not flow during <br />this period. <br />1 certify under penalty of law that I have personally examined and am familiar with the information submitted herein; and <br />based on my inquiry of those individuals responsible for obtaining the information, I believe the submitted information is <br />true, accurate, and complete. 1 am aware that there are significant penalties forsubmitting false information, including the <br />possibility of fine and imprisonment See 18 U.S.C. § 1001 and 33 U.S.C, § 1319. (Penalties under these statutes may <br />include fines up to $10,000 and/or maximum imprisonment of between 6 months and 5 years.) <br />Brian A.
