Cottony Division Director Natural Resource Trustee On May 3, 2004, the Division of Minerals and Geology received yow 112 Construction Materials Reclamation Permit Application package for the Hazeltine Mine, File No. M-2004-031. Preliminary review of the information received determined that the following items must be received before the Division can consider yow application as being submitted and technical review can begin: FXFiiRiT i. _ RerlamaNnn T'Ewrs (Rote ~_d_tzl~ (1) All information necessary to calculate the costs of reclamation must be submitted and broken down into the various major phases of reclamation. You must provide sufficient information to calculate the cost of reclamation that would be incuaed by the state. FXAiRTT N _ Cnnrre of Ta val Right to Fntrr ~Rnte 6_a_ta~~ T7te Division requests that you submit a copy of the Warranty Deed or a copy of the Vesting Deed that shows Aggregate Industries - WCR, Inc. has title to the property. Yow application will not be considered submitted until the information listed above is received and found sufficient to begin ow review. A decision date will be established 90 days from the date of receipt of all of the requested information. Additionally, if you have already published notice you will need to republish notice, after the Dtt'ision considers the application submitted. This notice must be published once a week for fow (4) consecutive weeks, starting within ten (10) days of the date yow application is considered submitted. We will notify you when you should initiate republication of yow notice. The final date for receiving comments is the 20th day after the fourth publication or the neat regulaz business day. You have sixty (60) days from the above date to submit all necessary documents that the Office needs for an application to be considered filed. If at the end of the sixty day period, the appliartion has not been determined to be filed with the Offiw, the Office shall dew the application and terminate the application file. TIIis letter shall not be construed to mean that there are no other technical deficiencies in yow application.
