Cottony <br />Division Director <br />Natural Resource Trustee <br />On May 3, 2004, the Division of Minerals and Geology received yow 112 Construction Materials Reclamation Permit <br />Application package for the Hazeltine Mine, File No. M-2004-031. Preliminary review of the information received determined <br />that the following items must be received before the Division can consider yow application as being submitted and technical <br />review can begin: <br />FXFiiRiT i. _ RerlamaNnn T'Ewrs (Rote ~_d_tzl~ (1) All information necessary to calculate the costs of reclamation <br />must be submitted and broken down into the various major phases of reclamation. You must provide sufficient <br />information to calculate the cost of reclamation that would be incuaed by the state. <br />FXAiRTT N _ Cnnrre of Ta val Right to Fntrr ~Rnte 6_a_ta~~ T7te Division requests that you submit a copy of the <br />Warranty Deed or a copy of the Vesting Deed that shows Aggregate Industries - WCR, Inc. has title to the property. <br />Yow application will not be considered submitted until the information listed above is received and found sufficient to begin ow <br />review. A decision date will be established 90 days from the date of receipt of all of the requested information. Additionally, if <br />you have already published notice you will need to republish notice, after the Dtt'ision considers the application submitted. This <br />notice must be published once a week for fow (4) consecutive weeks, starting within ten (10) days of the date yow application is <br />considered submitted. We will notify you when you should initiate republication of yow notice. The final date for receiving <br />comments is the 20th day after the fourth publication or the neat regulaz business day. <br />You have sixty (60) days from the above date to submit all necessary documents that the Office needs for an application to be <br />considered filed. If at the end of the sixty day period, the appliartion has not been determined to be filed with the Offiw, the <br />Office shall dew the application and terminate the application file. <br />TIIis letter shall not be construed to mean that there are no other technical deficiencies in yow application.
