Data Reporting (Continued) <br />b) Quarterly <br />Monitoring results obtained during the previous 3 months shall be <br />summarized for each calendar month and reported on division <br />approved discharge monitoring report forms, received at this office <br />no later than the 28th day of the month following the completed <br />quarter. For example, for the 1st quarter (January, February, and <br />March) the 3 monthly reports must be received at this office no <br />later than April 28.
