August 2002 -29- 003-2203 <br />• total of 27 out of the 120 nucleaz density tests failed to meet specifications. These azeas were <br />reworked, recompacted, and retested to meet specifications. The number of retests was not <br />used to determine the testing frequency. Holes made in the SLF during testing were filled by <br />tamping SLF into the holes. <br />4.2.b.3 Depth Verification <br />Ames drilled at total of 66 holes in the compacted SLF to verify that the minimum 12-inch <br />compacted depth had been achieved. Golder monitored the SLF depth verification tests and <br />documented results. Depth verification holes were filled with bentonite chips and water. All of <br />the 66 holes drilled into the SLF had compacted depths greater than 12 inches. The testing <br />frequency met the minimum specified testing frequency of two measurements per acre. The <br />Soil Liner depth verification results are presented in Table 14. <br />I . 4.2.7 Drain Cover Fill <br />Golder provided visual monitoring of DCF placement. One Golder monitor and at least two <br />Ames laborers were present monitoring each dozer, unless two dozers were within <br />approximately 50 feet of each other during DCF placement. Golder verified that wrinkles in the <br />geomembrane did not fold over, that PVC grade mazkers were placed on a maximum 50-foot by <br />50-foot grid and at grade breaks, and that unsuitable material was not present in the fill. Golder <br />continuously inspected geomembrane during DCF placement, and marked azeas for repair if <br />required. After fmal grading, Golder monitored removal of PVC grade markers. <br />On Wednesday, June 19, an Ames D9R Dozer damaged the Phase fVa geomembrane at panel <br />P-67. The dozer was stopped and the extent of the damage was identified (approximately <br />12 feet by 12 feet) by Golder. Ames exposed geomembrane in areas with less than 24 inches of <br />DCF for Golder observation.
