Reporting: Salt Creek Mining Company must submit a Discharge <br />Monitoring Report (DMR) on a quarterly basis to the Division. This <br />report should contain the required summarization of the test <br />results for parameters shown in Tables VI-4 and VI-5 and Part I.B.1 <br />of the permit. See the permit, Part I.B for details an such <br />submission. <br />C. Additional Terms and Conditions <br />1. Materials Containment Plan: On September 30, 1988, [he permittee <br />submitted an engineered spill plan. An update to the plan is <br />required to be filed within 90 days of the permit effective date, <br />detailing all changes which have occurred since the original <br />submittal. If no changes have occurred, a letter to this effect is <br />required. Far specific requirements, refer to Part I.E of the <br />permit. <br />D. Specific Compliance Requirements <br />1. Submissions to the Division: The following are specific compliance <br />items which require permittee action.
