During this period, Outtalls B (Outtall 003A), D (Outtall 005A), and E (Outtall 006A), discharged to Foidel Creek. No <br />permit excursions were noted for this period. Outfalls 003A and 006A Flowed each week of the period. Outfall 005A flowed <br />each week of the period with the exception of the final week. <br />A Quarterly Whole EfFluent Toxicity (WET) test was conducted for Outfall 005W in December 2003. This test indicated that the <br />effluent was not acutely toxic to Daphnia magna survival. No WET test was conducted for Outfall 007W during the fourth <br />quarter of 2003, as this site did not flow during the period. <br />Outfalls 002A, 004A, 007A/007W, and 008A did not discharge; and therefore had no chemical tests during June. Outfall <br />020A/020W has not yet been constructed, therefore sampling of this site has not occurred. Note that ponds A (Outfall <br />001A/001W), and H (Outtall 009A) have been deactivated following bond release, and have been removed from the newly <br />amended permit, effective January 1, 2003. In order to reduce the amount of paper work, the following certification will be <br />substituted for the actual DMR forms to cover the outtalls, which did not discharge. <br />I certify under penalty of law that 1 have personally examined and am familiar with the information submitted herein; and <br />based on my inquiry of those individuals responsible (or obtaining the information, 1 believe the submitted information is <br />true, accurate, and complete. 1 am aware that there are significant penalties (or submitting false information, including the <br />possibility of fine and imprisonment. See 18 U.S.C. § 1001 and 33 U.S.C. § 1319.
