M-96-031, RECEIPT OF 110 CONSTRUCTION <br />MATERIALS RECLAMATION PERMIT APPLICATION PACKAGE -INADEQUATE <br />Dear Mr. Sutherland: <br />On April 18, 1996, the Division of Minerals and Geology received your application package for <br />a I l0 Construction Materials Reclamation Permit for the Weimer No. 2 Pit, File No. M-96-031. <br />Due to Mineral Rules and Regulations changes which became effective January 2, 1996 and the <br />Colorado Reclamation Act for the Extraction of Construction Materials, Senate Bill 95-156, <br />which became effective July 1, 1995, preliminary review of the information received determined <br />that the following information must be received before the Division can consider your application <br />submitted and can begin the technical review: <br />1) The "Responsibilities as a Permittee" section on Page 3 of the application is required to <br />be initialed by the applicant or their agent. <br />2) For any exhibit requiring proof of mailing, initial proof of mailing consists of a copy of <br />the postal date-stamped white Certified Mail receipt. Prior to approval proof of receipt, <br />in the form of a copy of the signed and postal date-stamped green Certified Mail card. <br />must be received by this Office. Alternate proof of confirmation of receipt by the <br />appropriate agency could consist of a signed statement by a representative of the agency. <br />3) Rule 1.6.2(1)(bl -Affidavit of Pontine Notice at the Location of Mine Site: You must <br />post notifies at the location of the proposed mine site, of sufficient size and number to <br />clearly identify the site as the location of a proposed mining operation giving your name, <br />address, and phone number, and stating that you have applied for a mining permit with <br />the Colorado Mined Land Reclamation Board. Your notice must also state that anyone <br />wishing to comment on the application may view the application at the Montrose County <br />
