Rd/MP195 <br />Local Jurisdiction Doug1 ac <br />,~ Dist/Section/Patrol 1 ~S~ <br />DOH ~Permlt No. <br />Permit Fee: $75. 00 <br />Date of Transmittal .~-ZS-$~ <br />i <br />Keelin>z And Company (herein called permittee) is granted permission <br />to construct an access approach on the east side of State Highway I-25 Fr.Rd, a distance of -2000 <br />feet north from milepost 1 q5 also known as 195.4 ,for the <br />purpose of obtaining access to rnmmarr•{a1 .The access approach shall be constructed, <br />maintained and used in accordance with the State Highway Access Code, the terms and conditions of this permit, site <br />requirements noted below, this paragraph, and listed attachments. This permit may be revoked by the issuing <br />authority if at any time the permitted access approach and its use violate any of the terms and conditions of this permit <br />or the State Highway Access Code. The use of advance warning and construction sighs, flashers, barricades and <br />tlagmen are required at all times during access approach construction within State right-of-way in conformance with <br />the MANUAL ON UNIFORM TRAFFIC CONTROL DEVICES, Part VI. The Department of Highways, its employees <br />and agents shall be indemnified and held harmless against any action or damageto property orpersons sustained by <br />reason of the exercise and use of this permit. <br />SITE REQUIREMENTS <br />1 1. This access to be used only for commercial with the ADT not to exceed 60 movements. <br />2. The access width not to exceed 35 ft. and to have 50 foot radius on each side. <br />I 3. The new pavement shall slope on the same plane as the present pavement surface. <br />I 4. Compaction of sub-grade, embankments and backfill shall comply with the Divisions; <br />I current Standard Specificettions for Road and Bridge Construction. <br />5. Hot Bituminous Pa.velnent. shall comply with Section 401. <br />6. All new lanes and/or access construction shall be built to the following requirements: <br />10 inches of Class 1 ABC; 4 inches of Class 6 ABC; and 4 inches of Grade E Hot <br />Bituminous Pavement placed in 2 lifts or to an approved equivalent construction. <br />7. All construction shall be completed in an expeditious and safe manner aril shall 6e finished within <br />45 days from initiation. The permitted access approach shall be completed in accordance with the terms and <br />conditions of the permit prior to being used.
