Revised, December 7, 1992 <br />This plan remains essentially unchanged except for the following <br />changes: <br />1. 6900 North Well - Streeter Fill - During the summer of <br />1984, the shallow 6900 north well collapsed. It was <br />later determined that since this well monitored only a <br />small perched zone within the fill, reestablishment of <br />the well would not be necessary. Refer to the 1984 <br />Annual Report for additional details. <br />2. 6900 South Well - Streeter Fill - It was noted on March <br />21, 1986 that this piezometer well had also collapsed. <br />This well was reestablished in the fall of 1986. For <br />further details concerning this well, refer to the 1986 <br />Annual Report. <br />3. New Upper Good Sarino Creek Surface Water Monitoring Site <br />This site replaced the Upper Good Spring Site to allow <br />for mine expansion. Monitoring shall utilize any <br />~- equipment, structures, devices or methods necessary to <br />measure and sample accurately the quality and quantity of <br />surface water. Refer to Map 37 for site location. <br />These commitments have satisfied the requirements of Stipulation 1 <br />in the Findings document. <br />The proposed surface water monitoring plan includes monitoring <br />required under the NPDES Permit Facility Number COG-850017 issued <br />by the Colorado Department of Health. Colowyo will measure the <br />quantity and quality of any discharges from the permit area in <br />compliance with the NPDES Permit. <br />Colowyo will report the discharge in accordance with the Clean <br />Water Act of 1977 under the NPDES Permit on a quarterly basis; <br />therefore, Colowyo will plan to use the NPDES report for filing <br />with the Division. All monitoring data is submitted in an annual <br />report. <br /> <br />4.05-15 <br />
