Reporting -Active Mining Operations: The permittee will be required to submit an Annual Report, covering January I through December 31 of each year, on their compliance with the SWMP. The Armual Report will comain, at a minimum: a) A report on the facility's overall compliance with the SWMP. b) A summary of aU comprehensive stormwater inspections made. c) Results and imerpretation of any stormwater monitoring performed. The Annual Report will be due ro the Division on or before February IS of each year (see Part I.E.1 of the permit). 2. Reporting - Irtactive Mining Operations: The permittee will be required ro submit a Compliance Report. This report will contain, at a minimum: a) A report on the facility's Duna!! compliance with the SWMP. b) A summary or copies of the annual inspection reports, or the triennial certification by a Professional Engineer that the fadliry is in compliance with the permit. The report will be due to the Division ors or before February 15, 1999, and will cover the period of time from permit certification to December 3I, 1998 (see Part I.E.1 of the permit). 3. Reporting - Remediatton Projects: The permittee will be required to submit an Annual Report, covering January 1 through December 31 of each year, on the compliance with the SWMP. The Annual Report will contain, at a minimum: a) A report on the facility's overall compliance with the SWMP, including a progress report on remediation activities. b) A srtmmary of all comprehensive srormwatn inspections made. c) Results and imerpretation of any stormwater monitoring performed. The Annual Report will be due to the Division on or before February IS of each year (see Pan I.E.1 of the permit). All reports required for. submittal sha(1 be signed and certified for accuracy by the permittee. (See Part I.E.1 of the permit.) E. Monitoring Sampling and testing of stormwater jar specific parametns is not required on a routine basis under this permit. However, the Division reserves the right to require sampling and testing, on a case-by-case basis, in the evem that there is reason to suspect that compliance with the SWMP is a problem, or to measure the effectiveness of the BMPs in removing pollutams in the discharge. F. Retention of Records The permittee for active muting opertttions shall retain a copy of the SWMP required by this permit at the reining site for the duration of this permit. The permittee for inactive mining operations or mine-waste remediatiors sites shall retain a copy of the SWMP required by this permit, for the duration of this permit. For all types of operations, the permittee shall retain copies of all other reports required by this permit and records of all data used to complete the application to be covered by this permit, for a period of at least three years.
