It is vour <br />responsibility to complete and submit the Wefl Construction and Test Report, Form Number GWS~1 andlor the Pump <br />Installation and Test Repoli, Form Number GWS~32 These forms are available from our offices. The well must be <br />constructed and the pump installed in accordance with the well construction standards of the State Board of Examiners <br />of Water Well Construction and Pump Installation Contractors. <br />THE WELL CONSTRUCTION AND PUMP INSTALLATION RULES STATE THAT ALL WELLS OF THIS TYPE MUST BE <br />pISINFECTED AFTER CONSTRUCTION AND AFTER PUMP INSTALLATION. <br />THE WELL CONSTRUCTION AND PUMP INSTALLATION RULES ARE AVAILABLE FROM DMSION OF WATER <br />RESOURCES OFFICES FOR A FEE OF 53.00. The rules cover well construction, pump installation, disinfection, <br />abandonmern and contractor licensing regulations. ff you are constructing your own well or installing pumping <br />equipmern, it is recommended that you obtain a copy of these rules for reference. <br />WENS TO BE PLUGGED AND ABANDONED MUST BE PLUGGED IN ACCORDANCE WITH RULF 11 (SHOWN <br />BELOW) OF THE WELL CONSTRUCTION AND PUMP INSTALLATION RULES. A WELL ABANDONMENT REPORT, <br />FORM NUMBER GWS-9 MUST BE SUBMfTI'ED TO CONFIRM THE PLUGGING OF THE WELL THIS FORM lS <br />AVAILABLE FROM OUR OFFICES. <br />IULE 11 ABANDONMENT STANDAADS <br />1.1 General <br />1.1.1 The plugging and sealing of all wells and test holes is necessary to prevent cornamination of groun <br />water and the migration of water through the unused borehole. h is the ummate responsibility of the wE <br />owner to have a well property plugged and abandoned. The well construction cornractor is responsib <br />for notifying the well owner of the plugging and abandonment requiremern pursuant to this Rule 11. <br />1.12 Persons authorized to install pumping equipmern may plug and abandon wells which do not require tt <br />removal of casing from more than one aquifer or the ripping or perforating of casing opposite confinin <br />layers. <br />1.1.3 All materials used for backfilling shall be clean, free from cornaminarrts and chemically ineR <br />1_2 Urtcordmed Wells-Wells completed irno unconfined aquifers shall be abandoned by filling with eith~ <br />on-site materials, clean sand or gravel to the static water level, then with chemically inert materials 1 <br />the ground surtace. A permanent watertight cover shall lie installed at the top of the casing. The casir <br />may be cut off up to five (5) feet below ground level provided the watertight cover is welded ~ <br />permanently attached to the top of the casing and the hole is backfilled to the land surface. <br />112.1 Cathodic protection holes, dewatering wells, hor¢ontal drains, monitoring and observation hole <br />percolation holes, piezometer holes, sump pumps and test holes shall be abandoned either pursua <br />to Rule 11.2 or by removing all casing which was installed and by filling the hole(s) with drill cuttings <br />chemically inert materials to within five (5) feet Of the ground surface. The top five (5) feet of the ho <br />shalt be sealed with materials equal to or less permeable than the top foot of the surounding sots. <br />11_3 Confined Wells <br />11.31 Wells which were constructed through more than one aquffer shall be abandoned by placing a gro <br />plug at the confining layer above each aqurfer. ff records do not show that the casing opposite ear <br />confining layer has been grouted when originally installed, the casing shall be either completely removt <br />from the hole, or perforated or ripped opposite such layer prior to placing the grout plug. No plug sh, <br />be less than twenty (20) feet in length. <br />11.3.2 The well casing except for the grout plug irnervals shall be completely filled to the land surtace wi <br />chemically inert materials. A watertight cover will be permanently welded or attached to the top of tl <br />casing. The casing may be cut off up to five (5) feet below land surtace provided the watertight cov <br />is welded or permanernty attached to the top of the casing and the hole is backfilled to the land surfac <br />t <br />
