LODESTAR ENERGY, INC. <br />MUNGER CANYON MINE <br />IMPOUNDMENT YEARLY INSPECTION <br />In accordance with Rule 4.05.9(14), all impoundments shall be inspected at least yearly <br />to determine if the impoundment has been maintained as designed, and in accordance <br />with the approved plan and the applicable regulations. This yearly inspection is for the <br />impoundment located at the Munger Canyon Mine. <br />On March 26, 2003, I performed the required yearly inspections. I, J. E. Stover, have a <br />wide variety of experience in the design and construction of earth fill embankments. <br />Nothing was observed during the inspection that would indicate the pond has a <br />potential for failure. There was no appearance of erosion, instability, structural <br />weakness or other hazardous conditions. There are no required monitoring procedures <br />or instrumentation other than monthly and yearly inspections. <br />SAE-1 located at the portal is a sediment trap/impoundment. It has a required capacity <br />of 4,200 cubic feet. Sediment buildup in the impoundment has decreased its volume <br />however, based on rough survey data and available electronic topography, it was <br />determined the size of this impoundment meets or exceeds the required capacity. This <br />impoundment has been in place many years and has not shown signs of instability even <br />though the side slopes on the impoundment are at best 1.5H:1 V on both the inside and <br />outside slopes. These steep slopes do not meet the requirements of Rule 4.05.9(7)(e). <br />The steep slopes may affect the stability of this impoundment. The Operator plans to <br />replace this impoundment with a rock check dam and silt fence as soon as the area <br />drys. <br />With the exceptions noted above and to the best of my knowledge and belief, the <br />impoundment at the Munger Canyon Mine has been maintained as designed and in <br />accordance with the approyq~,'gha~~nd applicable regulations. <br />,U/ 90 <br />.e.a <br />^~ a?k` q o3 <br />m <br />E.
