1 Permit No. CO-0032751 <br />County: Boulder <br />AUTHORIZATION TO DISCHARGE UNDER THE <br />NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM <br />In compliance with the provisions of the Federal Water Pollut~on Control <br />Act, as amended (33 U.S.C. 1251 et. seq; the "Act"), and the Co loreido Water <br />Quality Control Act (25-$-101 et. seq., CR5,1973 as amended) ' <br />Hendricks Mining Company, <br />is authorized to discharge from its wastewater treatment facilities or the <br />Gross Mine Operation, <br />located on Cross Mine Road (TIS, R73W, Section 8 & 9) near Caribou, Colorado, <br />to receiving waters named Caribou Creek, <br />in accordance with effluent limitations, monitoring requirements and other <br />conditions set forth in Parts 1, 11, and III hereof. <br />This permit shall become effective thirty (30) days after the date of <br />receipt of this permit by the Applicant. Should the Applicant choo:~e to <br />contest any of the effluent limitations, monitoring requirements or other <br />conditions contained herein, he must comply with Section 24-4-104 CF;S 1973 and <br />the Regulations for the State Discharge Permit System. Failure to contest any <br />such effluent limitation, monitoring requirement, or other condition is consent <br />to the condition by the Applicant. <br />This permit and the authorization to discharge shall expire at midnight, <br />June 30.'1981. <br />Signed this day of <br />COLORADO DEPARTMENT OF HEALTH <br />Division of Administration <br />Robert D.
