A copy of this record must be given the employee at the <br />completion of the training received (i.e. New Miner Training, Refresher <br />Training, etc.) The other copy must be retained at the mine site. A <br />copy of the 5000-23 should not be sent to this or any other PiSHA office. <br />Additional 5000-23 forms may be obtained from this office as needed. <br />Please do not record Party 48 training on the 5000-1 forms. <br />~ / <br />Sincerely, , <br />Walter R.
