On July 16, 2004, your application for the above captioned permit was considered "filed" for purposes of
conducting a technical adequacy review. This filing date marks the beginning of the Division's 30-day
terns for issuing a permit decision. This letter constitutes the Division technical adequacy review, and
addresses itemsar~issues that need additional information or clarification.
Attached to this review is the Division's estimate of the cost for reclaiming this property. Tf you consider
any of the information herein to be incorrect, you should address your concerns in writing to the Division.
Please note. This is not a permit approval. Until all issues identified herein aze addressed to the
Division's satisfaction, and until we have received a bond sufficient to cover reclamation, you may not
begin excavation or disturb the property.
APPLICATION FORM:
Item 1.2.
You must famish the Tax ID number or your Social Security Number.
Item 3.
This item indicates 7 acres aze to be permitted. However, the mining and reclamation maps indicate the
mining area is to measure 300' x 300', or slightly more than 2 acres. Please clarify. Permitted acreage
should include all areas disturbed by nlilung including, but not limited to, the area from which material
will be removed, areas used for topsoil storage, and areas needed to reach the topsoil and overburden
during mining and reclamation. If the access road is upgraded or enlarged, it must be part of the permit
azea. You should permit the excavation azea plus enough room to store topsoil and overburden, re-grade
topsoil and overburden after mining, and store or stage product during mining. Topsoil must be cleared
from product storage areas. Please explain which area is to be disturbed, and revise the mining and
reclamation maps to show where topsoil, overburden, and product stockpiles will be stored.
