At all times, including periods of start-up, shutdown, and malfimction, the facility and control equipment shall, m the <br />extent practicable, be maintained and operated in a manner consistent with good ae pollution prevention and control <br />practices for minimising emissions. Determination of whether or not acceptable operating and maintenance procedures <br />ace being sited will be based on information available to the Division, which may include, but is not limited to, monitoring <br />results, opacity observations, review of operating amd maintenance procedcaes, and inspection of the source. <br />6. Construction / ModiScation of the sources tilted render AIRS POINT Ids: 158, 159, and 163, must commence whbin l8 <br />months of initial approval permit issuance or within 18 months of the start-up date stated in the application. If <br />commencement does not occur within the stated time the permit will expire on April 30, 1998. <br />(See General Condition No. 6., Item I on the reverse side of the fast page of this permit) <br />7. The activities at this facility shall be limited to maximum production /consumption /processing 1 operation /handling rates <br />az listed on Attacbmemt A. Short tens limits (per day) shall be a®ual limits divided by 143. Records of the actual ratest, <br />and emissions of criteria and noncriteria reportable / bamcdous air po0utams shall be maintained by the applicant and <br />made available to the Division for inspection upon request Such records shall, at the minimum, contain the derails given <br />in Attachment C. <br />8. Emissions of air pollutants shall not exceed the following limitations (az calculated in the Division's preliminary analysis): <br />Particulate Matter :..................... 159.0 toms per year and 2,224 pounds per day <br />• Particulate Matter < 10 µm (PM,~:......... 85.0 tons per year and 1,189 pounds per day <br />Note 1: In addition, the above activities also emit the following fitgitive dust, and these have been entered into the <br />Division's emission inventory system: <br />Particulate Matter -Fugitive: 319.0 toot per year and 4,448 pounds per day <br />Particulate Matter < 10 µm (PM,~: 83.0 coat per year and 1,161 pounds per day <br />Note 2: The above emissions are hazed on emission factors and control efficiencies given in Attachment D. <br />Note 3: The permittce shall identify emissions of nontTi6eriarepo~table / ha~dous as pollutants. If emissions of such <br />pollutants exceed the notificafian thresholds, Air Pollutant Emission Noticds shall be submitted within one (1) <br />month of the issuance of this permit <br />9. Compliance with annual limits shall be determined on a rolling twelve (12) month total. By the fifteenth day of each <br />month a new twelve month total shall be calculated using the previous twelve calendar months' data. The permit holder <br />shall calculate monthly emissions and keep a wmpliance record on site for Division review. <br />10. Prior to final approval being issued; the applicant shall submit to the Division for approval a revised facility-wide <br />compliance plan amd record kceping format that will outline how the applicant will maimtain on-going compliance with <br />the requvememts of this permit This includes the control efficiencies speciSed in Attachmem A. <br />I1.
