Outtall 003A
only discharged the first, third and fourth weeks of the period. Outta11005A discharged each week of the period. Outtall OO6A
discharged only during the first of the periotl, allowing only one sample set to be collected and analyzed. No exceedances
were noted.
Quarterly Whole Effluent Toxicity (WET) testing for OO5X was conducted at this site the week of September 10th. Results
indicate that the effluent was not toxic to fathead minnow or Ceriodaphnia dubia. Copies of laboratory test results are attached.
Note that new DMR carbon sheets we recently received in the mail indicated 003X under discharge number instead of 005X.
OOSX is the proper designation.
In order to reduce the amount of paper work, the following certification will be substituted for the actual DMR forms to cover
the outtalls, which did not discharge. These include 002A (Pond T), Pond C (Outtall 004A), 007A (Pond F), and 008A (Pond
G). Note that ponds A (Ouffall 001A), and H (Ouffall 009A) have been deactivated following bond release, and have been
removed from the amended permit, effective January 1, 2003. Also note that Outfall 020A has not been constructed yet.
CERTIFICATION
1 certi/y under penalty of law that I have personally examined and am familiar with the information submitted herein; and
based on my inquiry of those individuals responsible /ar obtaining the information, 1 believe the submitted information is
true, accurate, and complete.
