Modification Sus easion or Revocation of Permits B the Division <br />(Continued <br />(b) (Continued) <br />(111) Materially false or inaccurate statements or information <br />in the application for the permit; or <br />(iv) Data submitted pursuant to Part I.B indicates a potential <br />for violation of adopted Water Quality Standards or stream <br />classifications. <br />(c) The certification for the individual facility authorizing <br />discharge under the general permit may be modified to allow far a <br />change in ownership or opera tioaal control of a facility <br />including addition, deactivation or relocation of discharge <br />points where the division determines no change in the general <br />permit is necessary. <br />(d) At the request of the permittee, the Division may modify, or <br />terminate a certification if the following conditions are met: <br />(i) Ia the case of termination, the permittee notifies the <br />Division of its intent to terminate the certification 90 <br />days prior to the desired date of termination; <br />(11) In the case of termination, the permittee has ceased any <br />and all discharges to state waters and demonstrates to the <br />Division there is no probability of further uncontrolled <br />discharge(s) which may affect waters of the State. <br />(111) The Regional Administrator has been notified of the <br />proposed modification or termination and does not object <br />in writing within thirty (30) days of receipt of ' <br />notification; <br />(iv) The Division finds that the permittee has shown reasonable <br />grounds consistent with the Federal and State statutes and <br />regulations fot such modification, amendment or <br />termination; <br />(v) Fee requirements of Section 6.16.0 of State Discharge <br />Permit System Regulations have been met; and <br />(vi) Requirements of public notice have been met. <br />
