The <br />Mine Operator shall notify the Division of Minerals and Geology <br />(DMG) of a reportable spill within the mined land permit area <br />using the same timeframe required by the permit, .License, <br />notice, statute, rule or regulation governing tha reporting of <br />the spill to the other appropriate agency. Notice of a <br />reportable spill shall be FAXed to: Minerals Program Supervisor, <br />Division of L4ineral.s~and. Geology, F?Y (3^3) ^.'.~-OlOo'. T.~.e FAX <br />shall include a call-back number of a responsible company <br />official for DMG staff to use a contact. <br />No financial warranty was set by the Division for this operation. However, you must submit a performance <br />warranty in order for us to issue a permit. We have enclosed a performance warcanty form with this letter for your <br />use. Please have the performance warranty signed by an appropriate offical, notarized, and return it to us. <br />PLEASE NOTE THAT A4INING OPERATIONS MAY NOT COMMENCE UNTIL A PERMIT HAS <br />BEEN ISSUED BY THE'DIVISION AETEA RECEIPT OF YOUR PERFORMANCE WARRANTY. A <br />PERMIT CANNOT BE ISSUED- UNTIL WE HAVF.
