In that letter, well 88-1 was proposed as a subsidence monitoring well and <br />2A-2V was proposed for re-completion as a second raw water well. With the 1994 <br />approval of 88-1 as a subsidence monitoring well and the radioactive bullets in place (see <br />attached well diagram), NSI requests that 88-1 be classified as a subsidence monitoring <br />well. As a result of EPA and BLM approval, NSI is in the process of converting well 2A- <br />2V to a raw water source (see EPA approval, dated June 30, 2004). Prior to using the <br />raw water from 2A-2V, an approved well permit will be received from the State <br />Engineer's Office. In addition, a third well (4A-2V) listed for corrective action should be <br />modified from shut-into not active or not completed (see attached diagram). Vertical <br />well, 4A-2V, was drilled, cased and annulus cemented to surface. The horizontal <br />completion well 4-3H never was drilled to complete 4A-2V as a production well, <br />therefore it is not a production well requiring P/A. All aquifers are isolated and until a <br />horizontal well makes connection there is no potential mining. <br />During your site visit on July 26, 2004, you stated that Natural Soda should request a <br />permit modification for 88-1 to be used as a subsidence monitoring well, 2A-2V for use <br />as a second raw water well and 4A-2 V's change in status. NSI requests that the UIC <br />Permit reflect the above uses. <br />
