CO 80222-1530 <br />RE: PERMIT CO-0027154: APRIL 2002 DMR SUBMITTAL <br />Dear Ms. Lenkey: <br />RECEIVED <br />MAY 10 2001 <br />Oivisien of Minerals and Geology <br />Enclosed, please find the above referenced discharge monitoring report (DMR) far the period April 1, 2002 through April 30, <br />2002. During this period, ponds A (Outtall 001), T (Outfall 002), B (Outtall 003), (D (Outfall 005), E (Outfall 006), F (Outtall <br />007), and H (Outfall 009) discharged to Foidel Creek. Outfails 001, 002, 003, 006, and 009 discharged each week of the <br />period. Outfall 005 discharged during the first 3 weeks of the period, and OuttaU 007 during the first two weeks of the period. <br />Outfalls 004, and 008, did not discharge; and therefore had no chemical tests during this period. Outtall 020 has not been <br />constructed yet; therefore there is no discharge to enable chemical testing to date. Points 010, Of6 and 017 have been <br />eliminated from this permit. In order to reduce the amount of paper work, the following certification will be substituted for the <br />actual DMR forms to cover the outtalls, which did not discharge. <br />CERTIFICATION <br />1 ceRify under penalty of law that 1 have personally examined and am familiar with the information submitted herein; and <br />based on my inquiry of those individuals responsible (or obtaining the information, 1 believe the submitted information is <br />true, accurate, and complete. t am aware that there are significant penattres /or submitting false information, including the <br />possibility of fine and imprisonment. See 1B U.S.C. § 1001 and 33 U.S.C. § 1319.
