During this period, only ponds A (Outfall 001), B (Outfall 003), D (Outfall 005), and E (Outfall 006) <br />discharged to Foidel Creek. Outtalls 001 discharged the first three weeks of the period, Outfall 003 discharged during the <br />second, fourth and fifth weeks of the period, Outfall 005 discharged during the third, fourth and fifth weeks of the period, and <br />Outfall 006 discharged each week of the period. No permit excursions were noted for the period. . <br />Outfalls 002, 004, 007, 008, and 009 did not discharge; and therefore had no chemical tests during this period. Outfall 020 <br />has not been constructed yet; therefore there is no discharge to enable chemical testing to date. Points 010, 016 and 017 <br />have been eliminated from this permit. In order to reduce the amount of paper work, the following certification will be <br />substituted for the actual DMR forms to cover the outfalls, which did not discharge. <br />CERTIFICATION <br />I certify under penalty of law that /have personally examined and am familiar with the infomtation submitted herein; and <br />based on my inquiry of those individuals responsible for obtaining the information, I believe the submitted information is <br />true, accurate, and complete. I am aware that there are significant penalties for submitting false information, including the <br />possibility of fine and imprisonment. See 18 U.S.C. § 1001 and 33_U.S. C. § 1319.
