<br />Analysis requested <br />Type of preservative used, or "None" as applicable. <br />Dames & Moore job number <br />Labels will be affixed to sample containers prior to or at the time of sampling. The labels will <br />be filled out at the time of sample collection. The exact sample location and type of sample <br />will be recorded on either the Solid Substance or Liquid Substance Sampling Form. <br />All boxes used for archiving or sample storage will be labeled at the end and top with an <br />indelible marking pen. Box labeling will include the following: <br />• lob Number (e.g., 24436-00I) <br />• Location (e.g., Cazl Junction D.A.) <br />• Matrix (e.g., soil) <br />• Sampling Location (e.g., CC-1) <br />• Depth Interval, if applicable (e.g., 0-6 inches) <br />• Date (e.g., Apri125, 1993) <br />• Disposition (e.g., Archived) <br />Sample Seals <br />Sample seals aze used to detect improper handling of samples following sample collection up <br />to the time of analysis. Items such as gummed paper seals and custody tape will be used for <br />this purpose. Signed and dated seals will be attached so that they must be broken to open <br />either the individual sample containers or shipping containers. Seals will be affixed to <br />containers before the samples leave the custody of the sampling personnel. <br />Sample Transport <br />Samples will be packaged and shipped according to U.S. Department of Transportation and <br />EPA regulations. Samples will be delivered to the laboratory so that the requested analyses <br />can be performed within the specified allowable holding time. Samples will be accompanied <br />by the chain-of-custody record and sample analyte lists. Laboratory supplied chain-of~ustody <br />forms will be used. The analyze lists are prepared by sampling media and are provided in <br />Section 4.0 of the SAP. The analyze lists include the parameters to be analyzed by the <br />laboratory. The chain-of-custody form will indicate which analyze list the sample is to be <br />analyzed for and the total number and type of samples shipped for analysis. Authorized <br />laboratory personnel will acknowledge receipt of shipment by signing and dating the chain-of- <br />custody form and returning a copy to the Dames & Moore Project Manager. The laboratory <br />will also sign and date the air bill accompanying the samples and keep it on file with the <br />sample information. <br />
