PART I <br />Page 3 <br />Permit No. COG-850000 <br />A. COVERAGE UNDER THIS PERMIT <br />2. (Continued) <br />If the applicant does not receive a request for additional information <br />or a notification of denial from the Division within thirty days, <br />authorization to discharge in accordance wi[h the conditions of the <br />permit shall be deemed granted. <br />If the Division determines that the facility does not fall under the <br />authority of the general permit, then the information received will be <br />processed for an individual permit, and discharge will not be allowed <br />until 180 days after the date the application was received. The <br />applicant shall be notified of such a determination. <br />3. Authorization to discharge under this general permit shall expire on <br />June 30. 1997. The Division must evaluate this general permit once <br />every five (5) years and must also recertify the applicant's authority <br />to discharge under the general permit, at such time. Therefore, a <br />permittee desiring continued coverage under the general permit must <br />re-apply by January 1. 1997. The Division will de[ermine if the <br />applicant centimes to operate under [erms of the general permit. <br />B. EFFLUENT_ LIMITATIONS AND MONITORING REQUIREMENTS <br />1.
