Box 187, <br />Craig, CO 81625 <br />DATE: October 24, 1995 <br />PERMIT AND BAMPLE <br />• <br />NPDES #: CO - 0032115 <br />Sample Type: Effluent Grab - 001 <br />Sample Date & Time: 10-12-95/7:OOam <br />Test Date & Time: 10-13-95/11:OOam <br />Dilution Water: Reconstituted Laboratory Water <br />AND <br />Organisms: Ceriodanhni~ dubia and fathead minnow <br />Procedure: Tests were performed in accordance with the <br />most recent protocols specified by the Water Quality <br />Control Division of the Col.
